Home » Shockwave Therapy for Achilles Tendinopathy
A stiff, aching Achilles that flares up during activity and never quite settles is one of the more stubborn injuries an active person can pick up. When rest and standard rehab haven't shifted it, shockwave therapy offers an evidence-supported way to restart the tendon's healing process.
Achilles tendinopathy is a degenerative condition affecting the tendon connecting your calf muscles to your heel bone, most commonly presenting as pain, stiffness, and thickening either in the mid-portion of the tendon or right where it attaches to the heel. It's especially common among runners and other active individuals, and it's notoriously slow to heal — partly because the mid-portion of the tendon has relatively poor blood supply, and partly because what's happening isn't classic inflammation but a genuine breakdown of the tendon's collagen structure.
The most common presentation. Pain and thickening occur 2–6cm above the heel bone, in the tendon's "watershed" area with relatively poor blood supply. This is the presentation most commonly and successfully treated with shockwave therapy.
Pain occurs right where the tendon attaches to the heel bone, sometimes alongside a bony prominence. Can also respond to shockwave, though treatment technique needs to account for the tendon's proximity to bone.
Shockwave therapy's proposed mechanism directly targets what's driving Achilles tendinopathy: the acoustic pulses are thought to stimulate the formation of new blood vessels around the tendon, and to encourage more organised, structurally sound collagen as the tissue remodels. UK clinical guidance has recognised extracorporeal shockwave therapy as a treatment option for Achilles tendinopathy on the basis of its efficacy and safety profile.
Delivered along the affected section of the tendon using our BTL-6000 SWT radial shockwave device, covering the span of degenerated tissue.
Stimulates new blood vessel formation in a tendon area that's naturally poorly vascularised, improving the supply of nutrients needed for repair.
Encourages the tendon to lay down more organised collagen fibres, replacing the disorganised structure typical of chronic tendinopathy.
Combined with a structured calf-loading programme — typically eccentric or heavy-slow-resistance exercises — to teach the remodelling tendon to handle real running and jumping demands again.
Most patients complete 3 to 5 sessions, roughly a week apart. We'd generally suggest avoiding running or high-impact loading for about 48 hours after each session. Your return-to-running timeline is guided by how your tendon is tolerating load, not a fixed calendar date, since pushing too hard too soon on a still-vulnerable tendon is one of the more common ways this injury drags on.
Shockwave therapy tends to be worth considering for chronic Achilles pain that's lasted more than 6 weeks and hasn't responded to rest and a genuine period of structured loading. It's generally used for degenerative tendinopathy rather than acute tears, so a thorough assessment is an important first step.
